Augmented Mammoplasty (Breast Augmentation) is an operation to increase the size of the breasts using implants. The implants have a silicone shell that can contain either sterile saline/water or silicone liquid/gel. Incisions are made in inconspicuous areas to minimize visible scarring. Incision options include: Inframammary incision, Transaxillary incision, Periareolar incision. Incisions vary based on the type of implant, degree of enlargement desired, your particular anatomy, and patient-surgeon preference. After the incision is made, a breast implant is inserted into a pocket either: Under the pectoral muscle (a submuscular placement), or directly behind the breast tissue, over the pectoral muscle (a submammary placement). The method for inserting and positioning implants depends on the type of implant, degree of enlargement desired, your body type, and your surgeon’s recommendations. Incisions are closed with layered sutures in the breast tissue and with sutures, skin adhesive or surgical tape to close the skin. Over time the incision lines will fade.

Duration of the operation:
The surgery requires 2 hours to complete.

Post operative care:
  • Move slowly and get some rest. Avoid activities that tend to wear you out. Recline your had slightly elevated above lower body.
  • Early ambulation is required to speed up healing process and improve circulation.
  • Eat a light diet at the day of surgery. Begin with liquids and progress to soup initially then advancing your diet to regular foods.
  • You may take shower 24 hours after the surgery. Gently rinse the breast area with water and pat dry. When showering remove the gauze dressing but leave the steri strips in place.
  • Avoid strenuous activities that can cause pain and discomfort.
  • The incision will be covered with steri-strips and a small gauze dressing. It is normal to feel tired and sore for the next 2-3 days. It is normal to feel that the breast implants are not part of your body.  It takes 2-3 weeks for your beast implants to soften and feel more natural.
  • Bra: You do not need to wear a bra or may wear a loose fitting comfortable bra if desired. Do not wear a push up bra for 6weeks.
  • Wound Care: Remove the gauze dressing before showering at least 24 hours after surgery. Steri strips will remain for the next 6 weeks. Avoid direct sunlight to the incision for 1 year. Use a sunscreen with zinc oxide with SPF 20 or greater.
  • Medications: Take all medications as prescribed.  Follow all instructions which may accompany the medications such as: Taking with food, avoid alcohol and driving restrictions.
  • Follow up appointment: Your first visit will be scheduled to see Dr. Juta in the next 3-4 days.
  • Call Dr. Juta’s office immediately for: Sudden onset of pain, fever, nausea, and vomiting, bleeding, dizziness, shortness of breath, rash, rapid heart beat or rapid breathing.
Recovery period:
You may go back to work in 2-5 days depending on how you feel. 

Possible risk and complications:
The risks of breast implants and complications after breast augmentation include: unfavorable scarring, bleeding (hematoma), infection, poor healing of incisions, changes in nipple or breast sensation, may be temporary or permanent, capsular contracture, which is the formation of firm scar tissue around the implant, implant leakage or rupture, wrinkling of the skin over the implant, anesthesia risks, fluid accumulation, blood clots, pain, deep vein thrombosis, cardiac and pulmonary complications, possibility of revisional surgery.

Preparation before Surgery

  • Healthy lifestyle: maintain proper hygiene in weeks prior to surgery. Colds, cough, sore throat, virus or other illness will result in rescheduling of surgery. Make sure to address any illness immediately.
  • Exercise: Proper fitness and body stretching can help enhance posture and gives you more strength for the up coming surgery.
  • Get lab testing or medical evaluation: : Make certain all test ordered (CBC, EKG, X-ray) by your surgeon MUST be completed at least 5 days before surgery and no more than 30 days before surgery.  Test results should be received by Dr. Juta for clearance. If medical clearance is not yet received at the expected date of completion, surgery maybe delayed or cancelled.
  • Get a baseline mammogram before surgery and another one after surgery to help detect any future changes in your breast tissue.
  • Vital information: A preoperative visit or call is essential to review your health, your goals and any vital information including allergies and health considerations.
One week before the surgery:
  • Confirm your day of surgery: Review all orders and instructions and put your schedule together a few days before the surgery.
  • Continue to practice healthy habits: No smoking, no drinking alcohol and stop taking medications to ensure safety.
  • Relax: Get plenty of rest.  Anxiety is common over the thought of surgery, so don’t hesitate to contact our office.  We are here to support you and answer all of your concerns regarding surgery.
  • Shop for your post surgery bra: elastic cotton or other unpadded fabric with good full- cup coverage and support. Wide, well- balanced shoulder straps and wide supportive band across the back and beneath the breast with no under wires.
One day before surgery:
  • Pack your personal belongings for the day of surgery.
  • Arrival: You will arrive at Bangkok International Airport. You will be accompanied by Dr. Juta’s staff going to the hospital.
  • Assessment of the medical history (any allergies, serious medical condition and all medications taken both prescribed and non-prescribed), physical examination, and laboratory tests will be performed during consultation.
  • Nothing by mouth. No candy, gum or mints. Anything more than a small amount of water as needed for brushing teeth or swallowing medication may result in the need to cancel surgery.
  • Dress appropriately: do not wear jewelry, hair clips, body piercing, cosmetics.  (If there is something you cannot remove, let the admitting nurse know right away)
  • Wear comfortable, clean, loose- fitting clothing: Do not wear jeans or any tight fitting bottom; rather have a pair of loose, drawstring sweatpants to wear home. You may wear a robe. Wear slip on, flat shoes with a slip proof sole; no heels. Wear clean cotton socks as the operating room can feel cool.
  • Shower as directed: use an antibacterial, fragrance free soap. Shampoo your hair. Do not use deodorant or perfume. 
  • Do not wear cosmetics, jewelry of any kind, contact lens, hair clips body piercing: ( if there is something you cannot remove , let the admitting nurse know right away)
  • Relax: Rest and avoid unnecessary stress.

Liposuction, also known as lipoplasty, liposculpture suction lipectomyor simply lipo ("suction-assisted fat removal") is the most common cosmetic surgery operation that removes pockets of fats from many different sites on the human body. It can remove stubborn areas of fat that do not respond to traditional weight loss method but remember it is not a substitute for dieting and exercise. It also involves the use of a stainless steel suction tube called a cannula. The cannula is pushed and pulled through the fat layer, breaking up the fat cells and suction them out by a powerful suction pump, thus improving body contour and enhancing self image. Areas of the body where liposuction is performed are: abdomen, hips, outer and inner thighs, flanks, love handles, back, knees, arms, submental (chin), Gynecomastia.

  9. Do not apply hydrogen peroxide or plastic Band-Aids to incision sites. Liposuction incision sites heal faster and better without topical antibiotics.
  10. Do not soak in a bath, Jacuzzi, swimming pool, or the ocean for at least 7 days after surgery in order to minimize the risk of infection.
  11. Patient taking Aspirin and Ibuprofen for at least 3 days after surgery. NSAIDs promote increase the risk of bleeding by impairing platelet function and depress immune response to infection.
  12. Drainage is placed for about 3-7 days depending upon the extent of surgery.
  13. Patient must maintain a proper diet and exercise regimen because the remaining fat cells could still enlarge and form cellulite thus creating irregularities.
Recovery period:
Depending on the extent of liposuction, patients are generally able to return to work between 2 days-2 weeks. Bruising will fade after 1-2 weeks. Swelling will occur and increase 5-10 days after surgery and should subside gradually over 2 weeks- 2 months while numbness persists for several weeks.


Possible risk and complications:
With any type of liposuction procedure, complications may arise beyond the minor side effects but they are rare. Some possible complications include infection, allergic reaction to medication or material used during surgery, excessive fluid loss, thromboembolism, injury to the abdominal organs, and excessive loss of body heat (hypothermia), excessive intravenous fluids, aspiration pneumonia and cardiac arrest.
It is the surgeon’s responsibility to give the written list of signs and symptoms to watch for, along with instructions for post-op self-care.

However, because LipoSelection minimizes damage to other body tissues, patients typically experience less pain and fewer side effects than with other forms of liposuction. Serious liposuction complications are quite rare. Fat clots or blood clots may form, and there is a chance these will migrate to the lungs and cause serious illness. The fluid loss that results from liposuction will be anticipated by a qualified doctor, as it can cause low blood pressure, friction burns, or fluid buildup. The LipoSelection probe’s ultrasound energy may injure the skin or deeper tissues; although in most cases the benefits of the ultrasound system make it safer than traditional liposuction. If a doctor inexperienced with the LipoSelection procedure applies too much fluid, the wetting solution may collect in the lungs. If there is too much lidocaine in the liposuction solution, lidocaine toxicity may develop.
 
Before the surgery
At this time, you will be given an opportunity to ask any further questions. Continuation of healthy diet and exercise is very important before the surgery. Smoking must be stopped at least two weeks before the surgery
 
On the day of the operation
No eating or drinking for 8 hours before the surgery.  You should wear loose fitting clothing and sneakers. Do not wear sandals or slippers. You will meet with our nurse to prepare you for surgery. The surgeon will discuss the procedure with you again and do pre-operative planning. You will meet with our anesthesia provider to discuss anesthesia. You will be given a choice of day surgery or general anesthesia which required at least one day hospital stay depend on your choice but normally if it is an extensive surgery,   we required 4 days and 3 night’s hospital stay.
 
Anesthesia used
This procedure can be performed using local or general anesthesia.
 
 During the surgery
First, the area to be sculpted is filled with a special saline solution that helps numb the site and shrink the blood vessels (to minimize blood loss and reduce bruising). This solution wets and fills the area to be treated, making fat easier to break up with the VASER System’s sound energy. Then a small probe transmits sound energy (similar to that used for cataract removal in the eye) that breaks up fat on contact, while leaving other important tissues relatively intact. Finally, gentle suction of the sculpted site removes the emulsified fat quickly. Thanks to its patented technology, only the LipoSelection procedure first emulsifies and then removes fat in this manner.
 
Post Operative Expectation
The treated area will be wrap with compression garments snugly and continuously for 6 days-2wks. Usually drains are placed removing excess fluid over a period of 2-3 days.
 
 Back to normal work
Regular exercise work out may start after 2 weeks, start with light exercise then gradually move your way up to your regular routine as tolerated by your body.
 
Pre-Operative instruction
  1. Consultation with surgeon
  2. Evaluation for other medical conditions
  3. ECG and Blood test will be taken
  4. No intake of food or water 6 hrs prior to surgery
  5. Compression garments should be available to be used post-op
  6. Stop smoking and alcohol intake at least 2 weeks before surgery
  7. Anticoagulants, vitamins, hormones (including birth control pills), anti-depressants and some other medications also to be stopped at least 2 weeks before surgery.
     
Post-Operative instruction
  1. Wrap treated area with compression garments snugly and continuously for 6 days-2 weeks.
  2. A day after the surgery, patient may have light to moderate physical activities
  3. Some incisions will be left open and drainage is expected, absorbent pads should be used and changed frequently as needed.  
  4. Incisions will close by themselves within 2-3 days.  Use band aid to protect incision sites.
  5. Keep incision clean
  6. May have quick bath after 2 days post op. Regular shower when incisions are closed.
  7. May use regular girdle 2 weeks after compression garments.
  8. Do not apply ice packs or a heating pad to skin overlying the areas treated by liposuction
  9. Do not soak in a bath, Jacuzzi, pool or beach at last 7 days post-op
  10. Bruising may diminish after 2-3 weeks
  11. Return for check-up after 4 days removal of stitches.
  12. Swelling is expected, Maximal in the first 2 weeks, then gradually subsides.  Final result in 3-4 months.
  13. Skin may feel numb on the surface but insides are tender to touch.
  14. Some areas may become hard and lumpy; this is normal and will go away when fully recovered
